Can Early Orthodontic Treatment Help Avoid Tooth Extractions?
October 20, 2026
When parents see permanent teeth coming in crowded, one of the first questions is often whether their child will eventually need teeth removed to make room.
Sometimes extractions are the right choice, but not always. When crowding is evaluated while a child is still growing, there may be other ways to create or hold onto the space the permanent teeth need.
Why Crowding Happens
Crowding can have several causes. Some children have narrow dental arches or smaller jaws. Others have larger teeth, or the upper and lower jaws may not be developing in the same way. The timing of baby tooth loss and the way the permanent teeth are erupting can make a difference too.
Looking at those things early gives Dr. Bockow and Dr. Truong a better idea of where the crowding is coming from and whether anything should be done about it.

In some cases, the best choice is simply to keep an eye on things as more permanent teeth come in. In others, it may make sense to preserve space that is already there, expand a narrow upper jaw, guide erupting teeth, or address how the upper and lower jaws are developing.
Giving Teeth More Room
When treatment is needed, working with a child’s growth may help create more room for permanent teeth to come in. For some patients, that can lower the chance that teeth will need to be removed later just because there is not enough space.
That does not mean avoiding extractions at all costs. There are times when removing teeth can still be the healthiest and most stable choice. The important part is understanding what is happening before deciding how to treat it.
If your child has significant crowding, or you are wondering whether there will be enough room for all of the permanent teeth, an orthodontic evaluation can help answer those questions. Sometimes treatment makes sense now. Sometimes waiting and watching is exactly what is needed.
