Why It Happens and When to Have It Evaluated

Crowded permanent teeth are common, especially during the years when baby teeth are falling out and adult teeth are coming in. Sometimes several teeth are erupting at once, and things simply look crowded. In other children, there may not be enough room in the jaws for all of the permanent teeth to come in where they should.

When we see crowding, we look at how the jaws are growing, how much space is available, and how the permanent teeth are coming in.

Why Do Teeth Become Crowded?

Crowding occurs when the dental arches don't have enough room for all the permanent teeth to come in and settle into healthy positions. There isn't always a single reason for it.

Genetics can be part of the picture. Some children inherit relatively small jaws or narrow dental arches, while others have larger teeth. Sometimes it is a combination of both.

Jaw development can also affect the amount of space available. A narrow upper jaw or an underdeveloped lower jaw may leave less room for the permanent teeth. When crowding is more severe, a tooth can become blocked from erupting or become impacted.

Baby teeth matter too. If they are lost too early, or stay in place longer than expected, the permanent teeth may not follow their usual path as they come in. Prolonged thumb-sucking or pacifier use may also affect tooth position and the way the jaws and bite develop.

Will Crowding Improve as a Child Grows?

Crowding often becomes easier to see as more permanent teeth erupt. Although the face and jaws continue to grow, that growth does not always create enough extra room to resolve the crowding.

Much of the width of the dental arches develops earlier in childhood. Later facial growth tends to happen more in downward and forward directions, so waiting for a child to grow does not always solve the problem.

When Should You Have Crowding Evaluated?

An early orthodontic evaluation does not automatically mean braces or treatment. Sometimes the best plan is to watch how the teeth and jaws develop.

In other cases, treatment may help preserve space, address a narrow upper jaw, guide erupting teeth, or correct a developing bite problem before it becomes more established.
